ohorques Wellness Services is a community-based organization dedicated to providing comprehensive case management, behavioral health support, care coordination, and wellness services to children, adults, older adults, and families. Our team assists individuals in accessing needed medical, behavioral health, social, educational, vocational, and community resources that promote overall well-being and independence. Through person-centered assessments, individualized care planning, ongoing monitoring, and periodic reviews of service plans, Bohorques Wellness Services works collaboratively with healthcare providers, community agencies, caregivers, schools, and support systems to ensure coordinated and effective service delivery.
